Completion of this questionnaire is a required step in the check-in process for the 2026 California State Championship. All athletes and coaches must have this form completed prior to arriving at check-in. Those who have not completed the questionnaire will not be permitted to check in or weigh in until it has been submitted. The below information is requested by the City of Fresno.

The Heads of Team Meeting was held via Zoom on Saturday March 28th, 6 PM PDT.

Errata: The slide presentation had an error regarding the color of the mouth guards. The slides have been corrected, and the video now includes a voice-over correction. As stated in the information packet and consistent with current USATKD competition rules, mouth guards must be white or clear.
